    A) Community Health Worker

    Work Location

    West Vancouver

    Terms of Employment

    Full-time: Live-in/live-out Care, split shifts may vary

    Description

    The Community Health Worker (CHW) will assist clients within the community with all activities of daily living including personal care, light housekeeping, meal preparation and offer assistance with transfers and ambulation.  Companionship and respite care will also be involved.  Successful applications will be passionate and caring for others, patient and detail orientated and can ensure safety and comfort for a variety of different clients.

    Experience

    The CHW must be able to work independently in one on one situation with clients.  The position requires strong communication and personal care skills, a personable attitude and accurate record keeping skills.  Experience in care facilities or in clients’ homes is an asset.

    Qualifications

    PSW / HSW Certificate, First Aide and CPR, Clear Criminal Background Check, Valid Driver’s License, Updated Health Status Clearance, BC Care Aide & Community Health Worker Registry #

    **This position is primarily live-in, meaning that the applicant should be comfortable to spend the night in a client's home.

    B) Resident LPN

    Work Location

    North Shore & Vancouver

    Terms of Employment

    4 to 8 hour shifts, Days Vary

    Description

    The role of the Resident LPN is to educate and provide quality healthcare services to our home care and institutional clients.  While carrying out the nursing process, the LPN must utilize all techniques and interpersonal skills to promote the clients’ highest functioning ability and to enhance the quality of life for the client.

    The position includes performing in-home assessments, addressing the medical needs of clients in various in – home and institutional environments, and conducting follow up visits to ensure that care plans are executed effectively.

    Experience

    The position requires strong organizational and communication skills, the ability to work independently, while maintaining a passion for patient care within their community.

    On call work may be required.  Experience in community nursing and long term care would be an asset.  New Graduates are welcome!

    Qualifications

    Successful applicants must have graduated from an accredited institution of Nursing and must be licensed to practice with the provincial governing body.  A valid driver’s license, a clean criminal record check and current CPR are mandatory.  Successful candidates must show a strong commitment to superior customer service.
